Ksharas sutra treatment for anal fistula
Hello I had three tracks for anal fistula .for which i opted kshars sutra treatment. It was done on 2 spet 2025. After this ,every week I had gone through thread change process the pus is still coming. It's been more than 2 months. Now far from anal area the skin becomes hard and I can see hard lump is coming which is very painful.what will be the reason.? Since I have IBS issue as well so my ayurveda doctor advised me to eat khichdi and chaas Currently taking kutaj vati/ sanjeevani vati . How to know that my treatment is going better. Cause I have lost almost 12 kg since I am not eating nonveg and due to IBS issue my diet is restricted. pus is still coming out and one new lump is growing which is very painful. Please advise

I understand your concern; you’ve already gone through a lot. Pain, pus discharge, new lump formation after **2+ months of Kshar Sutra**, along with **IBS and weight loss**, must be very stressful. Let me explain clearly. --- ## 🔍 **Why is pus still coming? Why the new painful lump?** ### Possible Reasons 1. **Incomplete drainage of fistula tract** – Some internal extensions may still be left. 2. **Secondary fistula track forming** – A new abscess or track may be developing. 3. **Infection or granulation tissue buildup** – If cleaning or changing thread isn’t effective. 4. **Hard lump** could be: * Fibrosis (healing tissue getting hard) * Or **pus accumulation (abscess)** → if painful, likely **abscess** 5. **IBS & low immunity can delay wound healing.** --- ## 🚨 **Red Flags — You must get checked condition urgently if:** ✔️ Lump is **increasing in size** ✔️ Pain is **increasing** ✔️ Pus is **yellowish or foul smelling** ✔️ If mild fever or fatigue is present 👉 **This could indicate an active abscess**, and **may need incision & drainage (I&D)** OR reassessment of Kshar Sutra path. --- ## 🩺 How to know if your treatment is going well? | Healing Sign | Warning Sign | | ---------------------------- | --------------------------- | | Pain & pus decreasing weekly | Pus unchanged/increasing | | No new swelling | New swelling/lump | | Skin getting softer | Skin getting harder/painful | | Healthy granulation tissue | Red, swollen, hot skin | | Weight slowly improving | Continuing weight loss | ➡️ **Since you have new painful lump, weight loss & ongoing pus — healing seems delayed. You should re-evaluate with surgeon immediately.** --- ## 📌 Suggested Immediate Action ### 🔹 1. Revisit your Kshar Sutra expert * Ask for **MRI fistulogram or transrectal ultrasound** to check if there’s a new tract. * **Rule out residual infection or abscess**. ### 🔹 2. Continue Kshar Sutra ONLY after confirmation. If abscess is found → **may need drainage** before continuing thread therapy. --- ## 🌿 Ayurvedic Support (Along with Doctor Supervision) ### For Healing & Infection Control | Medicine | Dose | | -------------------------------------------------------------- | ------------------------- | | **Guggulu ****(Baidyanath)** | 2 tabs twice after meals | | ******Guggulu** | 2 tabs twice daily | | **Shudha *****(500 mg)** | Once daily | | ******Ras + *****Churna (local surgeon suggestion)** | Only if advised in person | ### Topical Application * ******Taila** for local application (around the region, not deep inside). * **Sitz bath**: Warm water + **1 tsp Haldi + 1 tsp Saindhava Namak (salt)** twice daily. --- ## 🧿 IBS Diet Support (Very Important for Healing) Continue **Khichdi + Chaas**, but include: ✔️ **Moong dal khichdi with ghee** ✔️ **Bilva fruit powder (Bael churna) 1 tsp before food twice daily** → Reduces IBS ✔️ ******vati + Bel churna together** work best ⚠️ Avoid: ❌ Spicy, fried, sour ❌ Wheat roti (gas forming) ❌ Cold foods --- ## 🍵 Healing Drink (Daily) Warm water + 1 tsp turmeric + pinch of black pepper – morning empty stomach. --- ## 💪 Weight Loss Management * Add **1 tsp ghee** in all meals * ******loha 1 tab twice** (helps anemia + weight + immunity) * If non-veg stopped, use **Moong dal, paneer, soaked almonds** --- ## 🧘 Lifestyle * Don’t sit long, use pillow * Walk slowly after food * Manage stress (IBS worsens with stress) --- ## 🔴 When to Seek Immediate Medical Attention ✔️ If lump becomes very painful ✔️ If pus increases or foul smell ✔️ If fever or chills ✔️ If openings multiply ➡️ **Please meet your Kshar Sutra surgeon ASAP to reassess. Delay may cause spread.** --- ## 🧿 Summary for You | Issue | Current Status | Recommendation | | ---------------- | -------------------- | -------------------------------- | | Pus still coming | Not improving | Re-evaluate | | New painful lump | Dangerous | Check for abscess | | Weight loss | Needs correction | IBS diet with strength herbs | | IBS | Stable diet but weak | Bilva + Kutaj | | Kshar Sutra | Ongoing | Continue only after reassessment | --- ## 🙏 Final Advice 👉 **Don’t panic; this happens in some complex fistula cases.** 👉 **Visit your surgeon this week for recheck imaging**. 👉 Continue Ayurvedic support but not without medical supervision. If you want, I can **prepare a complete IBS + Fistula healing meal plan with timings & herbs**. 📩 Would you like that? I can also help you prepare questions to ask your surgeon on next visit. Advice- self medication is hazardous, always consult ayurvedic doctor before taking ayurvedic medicine. Consult me personally for details.

The pus might be due to the infection caused during the ksara sutra procedure that commonly happens when not cleaned properly and while changing threads. The hard lump might be the inflammation caused after the surgery. Please make sure u do not get constipation. You should start eating more fibrous, light and laxative diet even if its pure veg food u will still be able to get rid of IBS if taken in a proper manner. You can start applying gandhak rasayana ointment or paste form on the part that has infected pus after proper cleansing, take EBH vati before bed once and u can even apply kailash jeevan ointment for reducing the ointment or castor oil from within the lump area.
