The first step should be to analyze the level of pain and correlate it with MRI scan. There are advanced options available now such as Platelet Rich Plasma Therapy. Many of my patients have benefitted from a combination of PRP and physical therapy. Surgery should always be the last option.

Permanent solution is neither physiotherapy nor is medicine It's surgery to replace the affected part of bone with international implant which will leave her pain free for atleast 25 more years

Permanant solution among the given option is physiotheapy, but it doesn't work after the damage is done So best option would be surgery, may be not right now, but ik few years.
