Since you had oral sexual exposure and subsequently developed persistent throat discomfort, it is reasonable to consider an STI involving the throat, although a red buccal mucosa and sore/dry throat are not specific for an STI. Reflux, allergy, post-viral irritation, mouth dryness and other ENT conditions are much more common explanations. Your ENT's assessment of acid reflux (GERD) therefore provides a plausible explanation. Pharyngeal gonorrhoea can occasionally cause sore throat, but many throat infections are completely asymptomatic.
Next Steps
If your previous âall STI testsâ were performed only from urine/genital samples, they would not reliably exclude an infection located in the throat. Because you had oral exposure, a pharyngeal (throat) NAAT/PCR for gonorrhoea is the most relevant additional test. CDC guidance specifically recommends testing the exposed pharyngeal site when there has been oral exposure. If your HIV and syphilis blood tests at 3 months were negative and there has been no subsequent exposure, those results are reassuring. Health Tips
Continue the reflux measures advised by your ENTâavoid very late/heavy meals, excessive spicy or oily foods, and lying down immediately after eating. Keep yourself well hydrated and avoid smoking/vaping and unnecessary mouthwashes or antiseptics, which can worsen oral irritation. If the throat symptoms persist despite appropriate reflux management, a review is appropriate. If you develop persistent ulcers, difficulty swallowing, neck swelling, fever or worsening pain, seek examination promptly. In short: your symptoms do not by themselves suggest an STI, and the reflux diagnosis is quite plausible. However, because the exposure was oral, one throat gonorrhoea NAAT/PCR can provide site-specific reassurance if it was never performed. Thank you
