I am a ckd patient for the past 5 years my creatinine was around 2.But 4 months back it rose to 2.5 then the Dr prescribed APLAZAR since taking it in a space of 2 months time my creatinine has been increasing ie 2.5 to 3.1 to 3.4 at the same time my urine protein has come down from 3++ to 2++.

Did you have any infection recently? Or any other medicine which you have taken.
If nothing new in the last 2-3 months, then most likely you have progression of your kidney disease. Unfortunately, CKD is often irreversible and progresses slowly.
You need to take care of your diet, blood pressure and hope that it stops progressing
