Your Uric Acid is on higher side and Vitamin D3 is low, hence you must be feeling joint pain also. Due to Uric Acid on high side. You will have to reduce Purine rich foods such as some pulses, red meat, spinach, cauliflower, sugary drinks, bear, coffee, peas etc.  Drink plenty of water at least 15-20 glasses of water to deal with High Uric Acid.  Allowed Foods are : Low fat dairy items, moderate intake of chicken -fish -eggs -tea-coffee -pulses. Nuts-Grains-Rice-Less sweet fruits & veg. Dietitian's advice is really good. Since Vitamin D3 is lower, you can buy Vit D3 from pharmacy or as advised by your Doctor. Get plenty of sunlight everyday along with enough dairy items to improve Vit D level. Your ESR level is little lower, hence you are getting explained symptoms. If you will follow these, majority of symptoms would gradually disappear. However advice of your Doctor's is beneficial as he may prescribe some types of medicines also.
Next Steps
Follow Dietary guidelines given as above buy Vit D supplement or Consult your Dr for Uric Acid medicine and Vit D supplement.
Health Tips
Most of the symptoms would disappear easily with proper dietary guidelines and proper medicins. Hence need not to be panic on this issue. If you need my advice, you can reach to me by Practo . Wishing you good health.
