People suffering from epilepsy can have behaviour problems or can have symptoms of mood disorder or changes in there personality otherwise called as epileptic personality. To confirm what exactly your brother is suffering from you have to get him evaluated by an astute psychiatrist and seek remedial measures. Good luck

In my opinion along with  medicines your brother also needs therapy which can help to eliminate grassroots problem which he might be having but is not able to face them, after due course of therapy he can lead a good life, and if someone is not able to form marital relation with their partner that is not introvert extrovert issue we need to fix this from where this dissociation is coming.
Next Steps
Seek therapist nearby or contact someone online.

Better to have a detailed evaluation by a professional to identify the reason behind his behaviour.
