My 4 year kid has been recently tested with hypothyroidism. Done testing with different good labs and have TSH of 7.4 and 9.6 but T3/ T4 are in range. He is otherwise active and no concerns except for feeling of tiredness after coming from outside sometimes and/ or leg pain.
Does he needs medication at this age?

Thyroid function tests varies with age group. However in given child TSH levels appears to be elevated. However starting thyroxine supplementation before finding out the root cause will be risky.
Child requires in person consultation for detailed evaluation
