The HSG needs to be repeated. If the uterine cavity is not opacified that means the dye didn't enter the uterine cavity. It has gone into the vagina because the HSG cannula got displaced.

Non opacification of uterus means the first film was taken late
No point repeating hsg if the tubes are visualised dye very well entered the uterine cavity
The report says both tubes are blocked at the fimbrial or distal end
Advised to proceed with hysterolaproscopy
