Please consult a gynecologist physically. If there is incompetent os, it would need to be sutured in future pregnancy. BMI is more, she can reduce some kilos for preventing further obesity related health issues
First — I am very sorry for your loss. A 19-week loss after everything looked normal is emotionally devastating. Your questions are valid. I will answer clearly and honestly.

From what you described:
• All genetic screenings normal
• Baby growth appropriate (361 g at 19 weeks is normal)
• Heartbeat normal
• Sudden loss around 19 weeks
• Diagnosis: Cervical incompetence (cervical insufficiency)
• Wife BMI: 36
This pattern strongly fits cervical insufficiency, not genetic abnormality, not semen issue, and not something you “missed.”
Why does cervical insufficiency happen?
It is usually due to:
• Structural weakness of cervix
• Prior cervical procedures (if any)
• Congenital collagen weakness
• Sometimes no identifiable cause
It is mechanical failure, not fetal problem.
Was BMI 36 the cause?
Obesity increases pregnancy risks (diabetes, hypertension), but it is not a primary cause of cervical incompetence.
So please understand clearly:
➡️ This was NOT your fault
➡️ Not because of semen
➡️ Not because baby was unhealthy
➡️ Not because tests were wrong
It is usually a structural cervical issue.
Next Steps
For next pregnancy:
✔ Cervical Cerclage (stitch)
Yes — cerclage significantly improves outcomes in proven cervical incompetence.
Types:
• History-indicated cerclage (placed at 12–14 weeks)
• Ultrasound-indicated (if cervix shortens)
Success rates:
• 80–90% carry pregnancy beyond viability when properly timed.
✔ Pre-pregnancy planning
• Weight reduction before next conception (target BMI
Health Tips
✔ Do not blame yourself
✔ Allow emotional recovery before next attempt
✔ Target weight reduction gradually (not crash dieting)
✔ Next pregnancy should be managed as high-risk from day one
✔ Choose a center experienced in cerclage
Many couples with cervical incompetence go on to have completely healthy babies with timely stitching.
This loss does not mean you cannot have a child — it means next pregnancy needs structured planning.
If you and your wife would like, I can help you design a clear pre-conception and next-pregnancy protocol step by step so you go in prepared and confident. Booking an online consultation will allow us to review her full history and give you a personalised, evidence-based plan.
I’m very sorry for loss of your baby.As a doctor I can explain what’s causing this if it helps you.
Cervical incompetence is a condition where cervix opens too fast before labor and unable to hold baby in uterus. Please note that it is not caused by you (hence all your semen parameters are normal) or any genetic/familial disorders.Since your wife had loss in second trimester it looks like cervical incompetence but not sure.
It’s caused by many factors like short/loosened cervix,previous surgeries/trauma like d&c,cervical procedures
And your wife’s increased bmi not exclusively causing this but minimal contributing like giving higher pressure on abdomen and more inflammation .
And also other factors like harmonal issues, stress factors may also cause this
Next Steps
If you are planning for another pregnancy please consult obstetrician about
-pre pregnancy consultation and planned cervical cerclage before pregnancy and go on with strict precautions and follow up properly. Many patients go on to have successful pregnancies with proper monitoring, progesterone support, and timely cerclage.
Take care

Very sorry for your loss. Cervix weakness can happen even when all reports are normal.
In next pregnancy, early check-ups and cervical stitch can help.
Dear Patient
I am very sorry for your loss. Losing a baby at 19 weeks is heartbreaking, but please understand clearly — this was not your fault or your wife’s fault.
From your history, the most likely reason is cervical incompetence (weak cervix). In this condition, the cervix opens early in the second trimester without pain, even when:
All blood tests are normal
Scans are normal
Baby growth and heartbeat are normal
That is why the loss happened suddenly despite a healthy pregnancy.
Your wife’s weight (BMI 36) is not the main cause. Many women with higher weight have normal pregnancies. Semen parameters being normal is also reassuring.
Future pregnancy — very good chances
A cervical stitch (cerclage) in the next pregnancy at around 12–14 weeks is highly effective. Success rate is about 80–90% when done timely with proper monitoring and progesterone support. Many couples go on to have healthy babies after this.
What to do next
•Consult a high-risk obstetrician before planning
•Start folic acid before conception
• Optimize weight gradually (helpful but not compulsory)
• Early pregnancy monitoring
Please keep hope — your reports show the baby was healthy, which is a very positive sign for future pregnancy success.
If you want, I can guide you about next pregnancy planning and precautions. 💛
