The xray ray you have shared it not very clear. The decay or dental caries is very close to the pulp of the teeth. The symptoms you have mentioned indicates that the pulp of the teeth is involved and infected. The peri apical regions is also showing signs of involvement of the tooth pulp. You have to visit a dentist and get a root canal treatment done on your teeth followed by a crrown. Porcelain or ceramic crown is advisable.

As per your description and x-ray there is tooth decay in your front tooth.
Your symptoms list are not complete and Xray is also not very clear, but it appears like it might need a root canal treatment.

From the given clinical radiograph, it seems that, there is deep cavity might be causing food lodgement & severe pain & get relief on taking cold water .
It requires endodontic therapy for relief.

Hello
The X ray that you have uploaded is a bit unclear but the hole in the truth is actually the decay which looks very close to the nerve of the truth. Plus the symptom of having pain in your tooth clearly suggest that the nerve is very likely to be involved. This tooth will most likely require a root canal treatment followed by capping.
