It looks like only dengue...high fevers with low TLC and platelet counts goes in favour of dengue. Patient does not need antibiotics, but only conservative treatment, with full hydration of body, paracetamol 600 regularly but not b4 3hrs after taking a dose, multivitamins. If platelets do decrease less than 20-25000, or if bleeding tendencies, platelets have to be infused . Tc. Stay admitted till fever in control, bcz rise of fever regularly means virus is active in body..it takes 3-5 days to settle, and few weeks to recover completely. Papaya leaf juiced or tab caripill, goat milk, are quite effective.

How are her urine cultures , also know is that platelets tend to drop in such condition with specific urinary infections , not very common though , but do check the same . Hope this was helpful .
It seems like a viral fever itself, which is most likely to be dengue itself. Kindly give her papaya leaf juice or papaya leaf extract tablets which has shown to be effective in increasing the platelet count. She will require symptomatic management as it's a viral disease. Keep her well hydrated drink atleast 3 liters of water per day.
