High esr of 110mm/hr
Hello doctor. My mother has knee on knee so doctor has suggested knee replacement surgery and for temporary relief we are going ahead with intraarticular knee injection. He asked to do certain blood test before that. ESR test result is 110 mm/hr although ref range is 0 to 20. He asked to consult physician to understand reason for high value. Pls suggest

There are so many causes . But it shows the inflammation somewhere in body may be in knee joins or anywhere else in body . So need to be evaluated further after adequate history talking.

High esr only indicates that there is some health problem since some time. It is not specific for any disease.

An ESR  of 110 mm/hr is significantly elevated. While ESR is a nonspecific marker, it indicates the presence of inflammation, infection, or systemic disease. Given your mother's condition and upcoming procedures, this high ESR needs to be evaluated further before proceeding with any intra-articular injection or knee replacement surgery. Possible Causes Osteoarthritis with secondary inflammation Rheumatoid arthritis or another autoimmune disorder. Chronic infection – such as tuberculosis, osteomyelitis, or a low-grade urinary/respiratory infection. Anemia, especially anemia of chronic disease.
Next Steps
Detailed history and physical examination by a physician or rheumatologist. Additional blood tests: CRP (C-Reactive Protein) CBC (Complete Blood Count) RA Factor, Anti-CCP ANA, dsDNA (for autoimmune diseases) Serum creatinine, LFTs Chest X-ray – to rule out tuberculosis or other chronic infections. Urine routine and culture – to check for urinary tract infections.
