I am attaching the USG report of my Grandfather's hernia (his age -90). My questions are -
1. Is is necessary to undergo surgery at this age for his hernia? (He recently had a Brainstroke and is on lot of medications for heart and lungs)
2. What are the complications if he doesn't opt for surgery?

He is unfit for surgery. You can use a hernia TRUSS instead which keeps the hernia reduced. Avoid excessive straining during defecation. You can give him laxatives.
Next Steps
Complications can be obstruction and gangrene of the gut loops if hernia becomes non reducible

Since he has had a stroke and is 90+ I understand the reservations about surgery. There are no alternatives to surgery but in special situations like this, there are options that can be used to manage the situation and avoid surgery.

Yes it is necessary to undergo surgery as the Hernia is large and containing gut loops. There is risk of the Hernia can getting complicated anytime. If the Herniated contents are obstructed or strangulation that will create an emergency situation which will lead to far more risk at this age if he had to undergo emergency surgery for complications of the hernia than an elective surgery for the Hernia.
Health Tips
Though surgery is high risk at this age still it is wise to undergo an elective surgery than an emergency one which can lead to risk of life.
