Second opinion with a cardiologist and physical consultation with orthopedist would most appropriate advise.

Get another cardiologist opinion followed by proper clinical examination with an orthopedic doctor. Headache, dizziness and chest tightness with left arm pain warrants attention.

It may be sometimes related to angina or  sometimes related to cervical spondylosis causing muscle spasm leading to pain.
Next Steps
You can start with hot fomentation,  take analgesic with muscle relaxant, take calcium and vitamin d supplement. You can also get one X ray of Cervical Spine to know if there are changes related to  Cervical Spondylosis.
Health Tips
You can also take a second opinion of Cardiologist because there were changes in ECG. So cardiac issue should also be ruled out..
