This is likely Recurrent Granulosa Cell Tumor... The Ureter will mostly be on the surface of the mass without infiltration... So surgery will be very useful in removing the same... Only in about 10% of cases, removal and joining of the Ureter will be needed... Other cases the mass can be removed without damaging the Ureter...
Next Steps
Opinion and management by a Surgical Oncologist to plan for surgery along with contrast CT scan of Abdomen to assess the mass and Ureter along the entire length...
Health Tips
Follow up with chemotherapy will likely be needed after surgery

Hello. It is a recurrent granulosa cell tumor and it is encasing the ureter with indistinct planes with blood vessels. I am not sure whether complete removal of tumor would be possible. Stenting of ureter and palliative chemotherapy and assessing for surgery at a later point would be a good option
Next Steps
a better option would be to try some chemotherapy and if planes with ureter and vessels become clear, then to attempt surgery.
Health Tips
the treatment is more of palliation in this setting and better not to attempt aggressive surgeries
