Are you doing regular back strengthening exercises? If there is a dynamic instability then surgery may be required.
Next Steps
Please visit an Orthopedic surgeon.

Does the pain stays in lower back region only or it radiates towards lower limbs as well.. Have you ever done any investigations for this pain.. Is there any instability while standing or walking.. There is a possibility of weakness in ligaments supporting the lumbar spine post delivery with a possibility of disc prolapse which might have increasedover the years.
Next Steps
You should use lumbosacral belt for support.. Take analgesic for pain relief, start calcium and vitamin d supplement.. You can also go for a fresh X ray of Lumbosacral spine to get the idea about the bony status. If there is some doubt, you may require to go for a fresh MRI to know the current status of the problem..
Health Tips
Avoid heavy work, avoid sitting down,  avoid Indian toilet, avoid forward bending..
