Hi. The best advice can be given only when you are examined and your ongoing medicines/ecg/echo are reviewed by a cardiologist. But still, if you are comfortable walking 3km and travelling by train, you seem to be ok to go to your office. But avoid too much of physical & mental stress. Your homocysteine is a bit high and you may need treatment for that. The creatinine is also high but if your urine output is good & you are not on any medicines harmful for kidney, then you may not worry much. Some iron supplement is likely to be needed.
