Foreskin issues

Hello doctors, this is in regards to my younger brother's issue. He is 24 years old. He complaints that while having sex his foreskin doesn't comes back n it hurts him alot though he is having sex on quite regular basis he has to apply some or other lubricants to insert in. Not sure what to do. Do we need to get him operated n cut the skin off ? If so what would b the time for operation n how fast he can recover and which clinic in Pune will b the best for the job ? Regards.

Phimosis refers to the inability to retract the distal foreskin over the glans penis. Pathologic phimosis defines an inability to retract the foreskin after it was previously retractible or after puberty, usually secondary to distal scarring of the foreskin. Paraphimosis is the entrapment of a retracted foreskin behind the coronal sulcus. Paraphimosis is a disease of uncircumcised or partially circumcised males....Up to 95% of cases of phimosis have been shown to respond to application of topical steroids to the preputial orifice, although some studies have suggested that this response rate may decline several months after the regimen is completed.
