It doesn't look all that bad.
But since you have symptoms it is better to consult with a General Surgeon in your area. He may advise removal of at least one nail completely.

I am seeing longitudinal parallel white ridges over the nail.. this might be fungal infection of the nails or even protein deficiency with liver disease.
