Active pus means the fistula track is still present. Scans like MRIs can easily miss tracks that are narrow or temporarily collapsed, but real physical symptoms like pus always take priority over imaging.
Because your original track was in the front at the 10 o'clock position, the swelling in your perineum indicates fluid is building up in that area. It can indeed travel forward toward the scrotum along natural tissue pathways if the drainage gets blocked.
Here is what you should do
Get an in-person physical exam by a Ksharasutra specialist or colorectal surgeon. A hands-on check is the best way to locate the internal opening right now.
Take warm sitz baths twice daily for 10 to 15 minutes to promote drainage and keep the area clean.
Plan for a procedural treatment like Ksharasutra or a fistulotomy. Medicines can temporarily reduce the pus, but a procedure is required to permanently cure the track from the root.

The MRI finding that the 3.8 cm fistula tract is no longer visible is reassuring. However, persistent pus discharge from the external opening means the fistula cannot be considered completely healed, even if the tract is not seen on MRI. Small residual tracts or intermittent infections may not always be visible, especially if the inflammation is minimal at the time of imaging.
The mild swelling in the perineal region without pain may represent:
Mild residual inflammation or scar tissue.
A small collection of fluid.
Persistent low-grade infection.
Based on your description alone, it does not necessarily mean that the fistula is extending to the scrotum. If the fistula were involving the scrotum, you would more commonly notice swelling, redness, pain, or a new opening or discharge over the scrotal skin. An examination by a colorectal surgeon is the best way to determine this.

Health Tips
Keep the area clean and dry, and continue warm sitz baths if they provide relief. Avoid constipation by maintaining a high-fiber diet, drinking adequate water, and avoiding excessive straining during bowel movements. Good hygiene can reduce irritation but will not eliminate a persistent fistula if one remains. Thank you
