Fever continuing for 7-8 days with severe headache and leg pain should not be treated only with painkillers - this needs proper evaluation. At her age with high BP and cholesterol, we must rule out dengue, typhoid, urine infection and any hidden focus of infection, and her long-standing NSAID use can also harm the kidneys.A fever lasting 7-8 days with severe headache and leg pain in a 50-year-old with high BP is not something to manage at home - she needs to be examined and tested today. Please get her seen by a physician and get a CBC, dengue and malaria tests, typhoid test, urine routine, CRP and blood sugar done; in this season dengue, typhoid, urine infection and viral fevers all present exactly like this. Till she is seen, give paracetamol for fever as advised, plenty of oral fluids and rest, and please avoid painkillers like ibuprofen or diclofenac on your own since she has drug allergies and high BP. Her BP medicines should continue, and note that persistent sweating, insomnia and severe headache also need her BP and thyroid checked. Take her to the emergency immediately if there is neck stiffness, drowsiness or confusion, breathlessness, chest pain, weakness of a limb, bleeding from gums or nose, red spots on the skin, vomiting or very low urine output. Once the basic reports are done you can book a consult with me on Practo and I will guide the treatment further.
