Hi,
Your current symptoms- worsening depression, increased anxiety, negative thoughts and especially suicidal thoughts- indicate that the present medication regimen is not adequately controlling your condition right now.
Please understand that this is not your fault; medicines often take time to work, and sometimes the combination or dosing needs adjustment by your psychiatrist.
Imipramine and quetiapine both take 2- 4 weeks for significant antidepressant effects, so one week is still early. However, any emergence of suicidal thoughts requires urgent reassessment.
Next Steps
1. Reach out for an online or in-person consultation:
📞 NEURONAL FIX CLINIC, RAMPRASTHA COLONY, Ghaziabad, (near Shreshtha Vihar, Delhi)- +91- 74XX52XX97XX67XX61
- Suicidal thoughts should never be waited out. We need to-
- We need to Adjust the dose of imipramine
- Switch to a stronger antidepressant (SSRI/SNRI) or add-on agent
- Review whether quetiapine 50 mg is sufficient
Reassess benzodiazepine (clonotril) use, as chronic use can worsen depression/anxiety
Health Tips
- Do not stop medicines abruptly, especially clonotril, as this may worsen anxiety.
- Avoid alcohol or substances.
- Ensure adequate sleep and keep someone informed about your mental state.
- Remember: these thoughts are a symptom, not a reflection of you — with proper treatment, they improve.

Next Steps
1. Start therapy immediately. A psychologist can help stabilise thoughts, provide safety planning, grounding techniques, and support while your medications are being reviewed. 2. Contact your psychiatrist right away. Your medicines may need urgent adjustment. Do not change doses on your own. 3. Ensure you are not alone. Stay with someone you trust and let them know how youâre feeling. 4. If thoughts intensify, go to the nearest hospital emergency without delay.
I can understand your distress
As per my opinion, you must wait for a week or at least 2-3 days more, these medications take minimum time of 1-2 week for showing their effect or response
Clonazepam can be taken for Sos basis for rapid control of anxiety
Do not worry, your symptoms will be improved, wait a week more, plan a follow up visit with your doctor,* make a record of your anxiety episodes duration frequency, discuss it with your doctor*
Sorry to hear that. Please discuss this with your treating doctor, alternative medication can be tried. Taking psychotherapy along with medication can be helpful. Medication can also take upto 6 weeks to show effects.
Initially for few days you can feel like this, and medicine effect will come in few weeks, you can take clonotril 0.5mg sos for anxiety . If symptoms are increasing see your psychiatrist immediately.
Consult your psychiatrist immediately. Consider inpatient treatment if suicidal thoughts are prominent. You can try rTMS after immediate crisis is handled.
