I have extreme perpetual pain in my left shoulder,
Absolutely no swelling no recent trauma no history of dislocation or severe injury due to any accident regular weight training till about last month.. however the issue has been there for quite a few years

As your prblm are from few years.
And recently mri shoulder shows partial thickness tear of supraspinatous tendon intraarticular. (inside the joint). This muscle help lift arm above your head .
2. there is Slap injury grade 2
3.inflammation of shoulder bursa ( fluid filled cushion like structure in shoulder act protection to bone muscle and tendon do not rub together)

1.Arthroscopy is key hole procedure to help fix above problm .
2. other option is taking intraarticular steroid injection. will relieve your pain but as your young and adventure do happen. it will reoccur. do best is to visit Arthroscopy surgeon and get it resolved. and post procedure after 6 weeks slowly phyio will be adviced. till your muscle get strong enough to start all activities.
From the information you provided, looks like you have partial thickness cuff tear with sub acromial bursitis
Simply put one of the tendons that form the rotator cuff is partially torn with degeneration along with some inflammation in the bursa in your shoulder
