My question is only a rabid dog can transmit rabies, so if I got bitten by a dog 5 months back does it have rabies ? if dog is still alive ? Should I need to take vaccination now?

The dog, which bites you 5 months ago, didn't have rabies at the time of the incident, that's why it is still alive even after 5 months. You don't have to get vaccination now.
