If u see x-ray properly you will find that the cavity is very near to the nerve area and it can infect soon so before it hurts you need to go for therapeutic filling by which we can save your teeth or if it is late then you have to root canal treatment followed by a cap and the gap between the two tooth can be closed by placing a proper size cap so that food particle doesn't get dislodged and can cleansable after gargling .... Thank you...

Clinically the cavity is usually bigger than what will appear in the x ray. This is because x ray is a 2 dimensional representation of the entire thickness of the tooth. Secondly you had pain in that tooth. These findings point towards a condition called irreversible pulpitis
Next Steps
Please get a root canal treatment/ endodontic treatment done for your 1 st molar tooth
Health Tips
Please don’t delay as the problem can escalate rapidly
Hi, you have deep cavity in your first molar tooth as well as there is interdental space between your both molar teeth. Root canal treatment with your first molar and joint crowns with both molars will be good treatment option. There are chances of food lodgement if you go for single crown with first molar tooth. If you go for joint crowns with both molars it will reduce percentage of food lodgement in the area between two molars.
