RTPCR CT value 21 means your wife has high viral load. She can breast feed her baby but while using mask and after washing hands. But still there are chances of spreading of covid.
So i would suggest to use formula milk for a few days until the viral load decrease and a CT value of more than 35 is attained. So that there is no danger at all. As babies are more prone to viral infections due to low immunity.
