With an old pangastritis on endoscopy and 8 months of heaviness, bloating, burping, nausea and poor appetite, this is functional dyspepsia with chronic constipation, and gas is a symptom rather than the disease. The leakage sensation usually comes from hard stool sitting in the rectum. Start isabgol 2 teaspoons in warm water at night, 3 litres of water and daily walking, a PPI before breakfast for 4 weeks, and a prokinetic if the fullness is bad. Cut tea on an empty stomach, smoking, alcohol and long gaps between meals.
Next Steps
Fibre, water, walking and a 4 week PPI course, plus CBC, TSH, blood sugar and a stool occult blood test. Since your endoscopy is 1.5 years old and symptoms have lasted 8 months with no appetite, a repeat scope is worth planning if there is weight loss, vomiting, black stools or difficulty swallowing, which need urgent attention. Book a consult with me with your old report and we will sort the constipation and the gastritis together.
