Cirrhosis on
My had has disbetes since 18 yrs and cirrhosis since 7yrs ultrasound done it shows 2-3mm tiny nodules which I read it's very small I read online do mri it's enchancement they're just 2-3mm how this can be issue doc said it's harmless I'm confused ultrasound attached

Hello,
The very small nodules are generally not dangerous, but in cirrhosis patient , regular monitoring is necessary. Spleen enlargement is common because of cirrhosis.
Next Steps
Looking at the reports I think further consultation is necessary. There isn’t anything dangerous but I feel it’s better to solve all issues step wise.

These nodules are present in cirrhosis, particularly in liver damage, caused by alcohol.
You can get a biopsy done to exclude hepatocellular cancer.

The ultrasound report here says that there are multiple 2 to 3mm nodules which may be suggestive of something known as "regenerative" nodules. So in cirrhosis of liver one of the changes that happens is that because of the constant changing of dead liver tissue into new tissue, nodules can form. Therefore the term "regenerative" because it's dead tissue trying to be thrown out and replaced.
So in this case what I would suggest is you monitor it closely and revisit the USG after 3 to 6 months and recheck the size of the nodule if it is growing or not. If the nodule is growing then I would suggest an MRI scan or a cytological study of the nodule.

Ultrasound is only the first line modality of imaging and though an enhancement as shown in the report sounds harmless, we need to consider the fact that he has cirrhosis which by itself is a precursor to hepatocellular cancer.
Next Steps
I would suggest you do an MRI with contrast to rule out the possibility of a cancer. And once it is ruled out he needs to be on annual surveillance USG just to keep an eye on any further changes or developments

2-3mm nodules are mostly benign in nature as per ACR categories. But still an MRI is advised to rule out any minor chances of Hepatocellular cancer.
