Burning at the back
I feel burning at the back mostly upper side of the body and sometimes middle and lower. I m taking sompraz 40 with evitol. I m having fatty liver grade 3. Sometimes indigestion is there. Burning is there since 1 month. Kindly provide treatment. And i dont want endoscopy. Is there any other test to diagnose the problem

The burning in your upper and middle back combined with indigestion is most likely GERD (acid reflux) — the acid irritates the esophagus and the pain often radiates to the back, which is very typical. Sompraz 40 is the right medication. If you don't want an endoscopy, a barium swallow study can give a reasonable picture of the esophagus and stomach without a scope. That said, your fatty liver grade 3 is something to take seriously — it significantly worsens GERD and causes its own discomfort. At grade 3, the most important treatment is lifestyle: reduce weight, cut out fried/fatty food, avoid alcohol completely if you consume it, and exercise regularly. Get your liver function tests and lipid profile done if not done recently. The burning will also improve significantly once the fatty liver is addressed.

Your symptoms may be due to acid reflux/gastritis, but persistent burning sensation over the back also needs evaluation for muscular, neuropathic or vitamin deficiency causes. Continue Sompraz 40 before breakfast and avoid oily/spicy food, smoking, alcohol, late meals and lying down after food. You may discuss adding a prokinetic/alginate preparation with your physician. If symptoms persist despite treatment, tests like CBC, LFT, blood sugar, Vitamin B12, Vitamin D, serum amylase/lipase, H. pylori testing and ultrasound abdomen may help. However, endoscopy remains the best test if symptoms continue, worsen, or there is weight loss/vomiting/difficulty swallowing.
