I'm wearing a boot on my left foot to help with Achilles tendon injury. I apparently strapped the toe portion extra tight and wore it all day and overnight. After taking it off , after 20 minutes I immediately felt pain. After 4 weeks I continue to have pain and the bunion area is painful to the touch and has a 2 1/2 inch area that's red and warm.

Probably you have torn your tendoachilles.
To confirm that you can get a ultrasound or MRI done.
If there happens to be a tear, you may have to undergo surgery for TA repair and reinforcement.
Less likely, If there is no tear, this might be Bursitis or Enthesopathy which can be dealt with medications.
