Most probable cause of bad breath is deposition of tartar on the teeth. If you do have such deposits, no frequent brushing or mouthwash would help. You simply need to get your teeth cleaned from a dentist. That means you need to get scaling and polishing done. Once the teeth are clean , the bad breath would go.
