Hello, I’m sorry your family is going through this. I’ll explain this step-by-step. Your mother has already received definitive treatment for breast cancer (surgery + chemotherapy + radiotherapy). The recent PET-CT showing involvement of supraclavicular (collar bone) lymph nodes suggests disease recurrence or progression. Yes, systemic therapy is appropriate in this situation because lymph node involvement outside the original surgical field indicates that the disease is systemic, not just local. However, before starting chemotherapy, a few important steps are strongly recommended: 1. Repeat biopsy – VERY IMPORTANT PET-CT shows metabolic activity but cannot confirm cancer on its own 2. Repeat IHC (ER, PR, HER2) Tumor markers can change at recurrence: 3. BRCA testing (if not done earlier) A positive result can open options like: PARP inhibitors.
