Hello ma'am, it's a sinus tract (abscess) ( from the photograph tooth is decayed). X ray of the tooth is needed. If she had gone for pulpectomy multiple times and still it's not working then go for extraction (removal of tooth) followed by space maintainer. Before going for extraction proper clinical and radiographic evaluation is necessary. Visit the clinic.

Hello It looks like the tooth is decayed, and if multiple treatments were done on the tooth and they are not successful it’s better if you go for the tooth extraction Better treatment plan can be given after seeing an x-ray

Hello This boil and pain is due to grossly decayed deciduous molar tooth. There is infection in this tooth causing pain. Clear xray is needed.
Next Steps
Visit a good Pedodontist or Endodontist nearby. Extraction of this tooth will be a last resort if it's not responding to pulpectomy treatment.
Health Tips
Brushing twice a day is advised for maintaining proper oral hygiene.
