For an age less than 40 with blood in semen has no problem.
But as it is painful, it can be due to infection, for which investigation is required. Below are the investigations:
Chlamydia NAAT (Molecular testing) - First-catch urine or urethral swab
Gonorrhoea NAAT / Bacterial Culture - First-catch urine or urethral swab
SyphilisSerology (RPR, VDRL, Treponemal tests) - Blood sample (or lesion swab if sore is present)
HIVAntigen/Antibody Immunoassay - Blood sample (or rapid oral swab)
Genital Herpes PCR Assay or Viral Culture - Swab of an active blister or sore
Trichomoniasis NAAT or Microscopy - Urine sample or urethral swab.
