Hi Your child is having decay tooth (more in a aggressive pattern) known as Early Childhood Caries(ECC) It affects both front and back teeth
Next Steps
Consult a dentist to treat the decay tooth by either extraction(removal) or filling depending on extent of decay
Health Tips
Once the above treatment has been done Follow a diet chart(based on sugar intake) prescribed by dentist to prevent decay in unaffected teeth and topical fluoride treatment(like l fluoride in toothpaste) professionally applied by dentist to protect the natural teeth.

Your child has a severe form of tooth decay that affects children calles early childhood caries. He needs to be treated otherwise all the teeth in the jaw will end up with cavities. The front teeth are already looking quite badly affected. Please take him to a pediatric dentist for management as well as advised on the changes you'll need to make in his diet and oral hygiene measures.

visit a pediatric dentist
