Bipolar disorder can be rated by scales such as YMRS but those need to be done by a psychiatrist. You need to see your psychiatrist in person.

There are different scales available for assessing the severity, but why is it that you want to know ? Your medicines sound the right medicines for the disorder and you are maintained on low doses, which is a good thing. There are also certain therapies which might help.
