Anxiety and depression
Obsessive thoughts, suicidal thoughts and reckless thoughts in mind. I am usually careless and jump from one task to other. Hired a therapist, took 5 sessions but it didn't work.

You need to consult a clinical psychologist or psychiatrist for evaluation. Based upon assessment you will be advised accordingly. Mere describing the problem and expecting tips for short cut solutions won't work.
Next Steps
consult a clinical psychologist or psychiatrist.

Hi... Please consult a Psychiatrist if the frequency and intensity of your suicidal ideations are high. Typically therapy must help but if you have taken therapy and not found it helpful. It will be better you consult a Psychiatrist first. In your case Cognitive Behaviour Therapy (CBT) will work well. Do find a good Psychologist when you are ready to try a new therapist. When you start taking Counselling Sessions from a Psychologist, come with a mindset of 15-20 sessions minimum taken on Weekly basis for 45 minutes duration each. Therapy works best in the longer plane of time. Therapy in your case may extend from 6—8 months to 1 year, depending upon the severity of your condition.
Next Steps
Consult a Psychiatrist and a Psychologist.
