Anus tight due to fissure

Asked for Male, 28 Years

I have anal fissure and due to this my anus remain tight. Doctor prescribe nifedipine ointment for this but its dont Working. Can you please tell why this happen? It is due to loss of blood supply in anus or due to inflammation of fissure. Or any other other reason.
