Until you do ultrasonography, Fnac and thyroid function tests..diagnosis of swelling is not complete and so is the treatment plan. First get these diagnostic tests done and follow up with reports

Ultrasound , guided FNAC  and Thyroid function tests to be done. However surgery is a must, the extent of surgery will depend on your investigative findings. But thyroid pathologies generally have very good prognosis. So please get the investigations done first.
