There can be several reasons why a tooth cap (crown) may come off frequently after a root canal treatment due to poor retention caused by many factors like poorly fitted crown, improper bite clearance, poor cementation, tapered tooth preparation, caries of underlying tooth structure, bruxism or night grinding of teeth. Ask your treating dentist to evaluate the situation and determine the underlying cause. In some cases, the crown may need to be re-cemented or replaced. Your dentist may also examine the tooth and surrounding structures to check for any additional issues that need to be addressed like adding retentive features to the tooth preparation.
Next Steps
It's worth noting that each case is unique, and the appropriate course of action will depend on the specific circumstances. Consulting your dentist will help determine the most suitable solution for your situation.

Hi as mentioned if the cap is coming out frequently it must be a retention issue. Reasons may be various: Inadequate tooth structure Ill fitted crown/ cap Improper bite Poor oral hygiene maintainance Poor gum health Kindly get an assessment and change the crown as the root canal treated tooth needs protection from forces of occlusion.
Next Steps
Ask your dentist to provide more retention grooves so that the crown holds onto the tooth structure well. Since the tooth is root canal treated getting an endo crown is also an option.

There are multiple reasons for this like inadequate crown structure, more angled walls during crown cutting, eating more sweet sticky food items etc. Do Visit a local Prosthodontist for physical evaluation and treatment (as required)

If your tooth cap is falling off frequently, even after undergoing a root canal treatment, there could be several reasons for this issue. Here are some possible causes and solutions: Poor crown fit: The crown may not be fitting properly on your tooth. This could be due to factors such as incorrect shaping, sizing, or bonding. It's essential to visit your dentist and have them assess the fit of the crown. They may need to adjust or replace it to ensure a proper fit. Inadequate bonding: The adhesive used to attach the crown to your tooth may not be strong enough or may not have properly bonded. Your dentist can evaluate the bonding process and reapply a more effective adhesive if necessary. Tooth structure issues: If there are any underlying problems with the tooth, such as decay, cracks, or insufficient tooth structure, it can affect the crown's stability. Your dentist will need to examine the tooth and address any underlying issues before replacing the crown. Biting or chewing habits: Certain habits like grinding or clenching your teeth can exert excessive force on the crown, causing it to loosen or fall off. Your dentist may recommend using a mouthguard or making lifestyle changes to prevent these habits and protect the crown. Poor oral hygiene: Inadequate oral hygiene can lead to gum disease or tooth decay, which can affect the stability of the crown. It's crucial to maintain good oral hygiene practices, including regular brushing, flossing, and dental check-ups, to prevent any underlying issues. Crown material selection: The type of crown material used may impact its durability and longevity. Discuss with your dentist the best crown material options suitable for your specific needs. It's important to consult your dentist regarding this issue. They will be able to assess the underlying cause and recommend the most appropriate solution, which may involve adjusting the crown, using a different adhesive, or addressing any underlying tooth issues.
