Severe pain : Herniated discs L4-L5-S1

I've had herniated discs since 2018, the pain went away with Physiotherapy and I was almost normal for a few years until last month when the pain suddenly increased and now it is untolerable. I can no longer stand or walk without the pain bringing tears to me eyes, the only position where the pain is bearable is when I'm lying on my stomach (on the bed) with knees slightly bent. Physiotherapy worked in 2018 but it has been almost useless this time, and I've been taking the following medication as advised by my doctor : Enzomac Forte : Twice a day Zerodol CR : Once a day Uprise D3 : Once a day Absolut 3G : Once a day He also advised to start Resner Plus once a day before bed, when there was no relief, but that made no improvement at all. Now he is recommending Medrol 8mg twice a day for 5 days, but my family has a history of diabetes so I'm scared of this medication, and I'm contemplating other options. Surgery would ALWAYS be my last resort. Any recommendations for pain mgmt ?

When the pain increased suddenly, better to have mri. Re evaluation by the orthopaedic doctor

Even I wont advise oral steroids which do have lot of side effects. Its quite common to have exacerbations in back oain with diac herniation from time to time. Almost 80% patients can be managed by medicines but if they dont work then root block injections is next logical step.

Disk herniation is of 4 types, without your MRI films it will be difficult  to answer. Short course of medrol won't affect your sugar levels that bad. You can start having  resner plus twice a day. You don't have improvement, because  you have disk herniation and that is affected you badly. I'm Dr Balaji, I have experience  of 11 years. You can take a consultation so that you will be able to understand  more.

Its better idea to go for local steroid by root block rather than oral steroid So ideal thing is to go for transforaminal epidural injection

Rather than going for oral steroids, opt for epidural steroids which acts at exact location of problem. Visit nearby pain specialist.
