Hiatus hernia

Dear Dr, I.m suffering from.hiatus hernia from.2017 but recently from.last 2 weeks I m feeling like something stuck in throat and something stuck in chest after waking up and some times metallic taste in mouth. I done endoscopy last week it shows Large Hiatus hernia and OG junction 40 cms . So do I need surgery for this or it can be manage with medicines? And in what stage we need to do compulsory surgery Thanks, Rashid
