Hello sir, By looking at your reports, serum ferritin is low that means your mother most probably has iron deficiency anemia. Hemoglobin this low cannot rise with any elemental iron tablet or iron sucrose injections. You need to transfuse at least 2 units of PRBC, and then again again get a CBC done. She definitely required blood transfusion as it is too much less compared to normal female hb level should be 11 to 13 g/dl and she needs iron supplements and through diet iron rich food like green leafy vegetables, ground nut etc and the cause for anemia should be ruled out as just blood transfusion and iron supplements not permanent solution until and unless cause treated. We have to ask pt about bleeding through anus,or any history of heavy menstual bleeding with irregular cycle even though her age is more than 50 she may have attained menopause we have to rule out postmenopausal bleeding like endometrial polyp , cause cervix, PID etc

Yes blood transfusion is recommended when haemoglobin level less than 7-7.5. Follow up investigation required for low haemoglobin, source of blood loss or nutritional deficiency. Continue taking haematinics after blood transfusion.
