Raised ESR points towards infection/inflammation. Also,there can be many reasons of low Hemoglobin and platelets, including anemia, dietary deficiencies etc. Kindly consult, and inform about your symptoms more. Do a proper blood profile along with LFT too.

An elevated ESR is a nonspecific sign of inflammation; it does not diagnose one disease by itself. Low haemoglobin with low platelets needs prompt evaluation for nutritional deficiency, infection, liver/spleen disease or bone-marrow causes. Please get CBC with peripheral smear, reticulocyte count, iron/B12/folate and liver tests, and review the reports with a physician. Your decreased hb with mcv being higher side of normal shows megaloblastic anemia which will be due to vit B9 or vit B12 deficiency and raised ESR indicate possible ongoing infection
