Advice on ultrasound and CBP
Dear Sir / Madam, 2 months back, we have found lymph nodes on my son neck. we consulted ENT doctor and she has  given 5 days antibiotic and pain reliever. After that as adviced by docter we have done with neck ultrasound and CBP Please find the below ultra sound report . Bilateral parotid and submandibular glands are normal in size & echotexture.  There are small volume intraparotid lymph nodes of short axis less than 3 mm. No other focal lesion seen. There are small volume bilateral cervical lymph nodes with preserved fatty hilum in level II, III, and V, largest measuring 17 x 6.4 mm in right level II and 16 x 5.6 mm in left level II. IMPRESSION :* Bilateral cervical lymph nodes - Likely reactive nodes. He do not have any cough , could and fever No weight loss, no Appetite loss, no night sweats Seeking your advice on the above:  Is it really require any FNAC and Biopsy. Attached CBP

One should correlate with other signs . Lymph nodes have persisted for 2 months . If consistency is firm, matted etc , then it immediately requires further investigations. However in other situation- start with a course of antibiotics and review after a week to see whether they have regressed. If in doubt - a FNAC or biopsy of lymph node would be done.

Dear concerned, You have done a lot of research and got nearly all tests done. If the USG is suggestive of reactive nodes and there aren’t any clinical signs of lymphoma which you have elaborated in detail. Looking at CBC I can see a leukopenia with good lymphocyte count. This may point towards a recovering viral illness. My advice will be to wait for another three to four weeks and repeat a CBC ESR USG NECK. IF needed, you can continue the same antibiotics for another five days. The chances of lymphoma is very remote in the given clinical situation. But we need to wait for a month to come to a conclusion.
Next Steps
CBC ESR SMEAR AND USG NECK after three to four weeks if no new symptoms like fever.

I think there is some issue over his scalp also.
Next Steps
ar this moment no biopsy or FNAC. As discussed previously a repeat USG and CBP after 2 to 4 weeks woth watch on any rapid lymph nodes size increase, unexplained fever, excess sweating , reduced appetite.
