Hi grade 1 tear is usually insignificant but as you are in difficulty, it should be examined properly, because straight away surgery is not good

I would suggest that you go for Arthroscopic ACL reconstruction as your problem is 1 year old now . Also thigh muscle bulk would have reduced due to differences of walking abilities over injured knee.
Health Tips
Quality of repair might not be of good quality of strength in your case that is y problems are persisting.

Grade 1 tear doesn't require surgery. It can be very well managed by Physiotherapy. Go for mobilisation and strengthening exercises tailored as per your clinical status.
