Your Echo report is suggestive of concentric LVH which is a indicator for elevated Blood pressure which is not controlled adequately
Issues of Mitral valve are age related and MR grade 2 needs yearly monitoring with Echo test
Next Steps
It is advisable to increase the dose of anti hypertension medications
