My dad had oral exam today doctor told he has white patches in mouth but my dad said that it is been there for 8-9 years actually first it was first in all over mouth and it was paining while chewing but now it is only on one side doctor is saying keep a look for its changes like if it becoming red what does he mean and he said go for biopsy if you want to be on safer side my dad denied that and he was confused what could be caused behind it he never chewed tobacco and he did smoke but it was only for one year and it's been 10 years since he left it what could be the cause behind it? I'm scared what did doctor mean by red? Red means cancer? And is this pre malignant condition?

2.habit of pan Gutkha
3.any prosthesis in mouth for long term
4.diabates.
As you told that there is white patches but you did not mentioned particularly in which region? Tongue , inner side of cheek, lips or other.
White patches are not always harmful.If you don't maintain proper hygiene it nay be.
Leukoplakia is a common condition where you can find like this.Its not malignant.
It's a pre malignant lesion.
Next Steps
you can do one thing in your own.
took a piece of cotton soak it in water are squize it and rub it to the affected area.If you see any visibility of blood thn you have to worry.
Thn it can be told(not surely) it's not leukoplakia , it's oral lichen planus.Which is a pre malignant condition.
As you also mentioned that there is pain so the direction of thinking is going there.

Stop all the habits.If have.
if there is any I'll fitting dentures remove it.
