a physiotherapy chart basically should contain the precise assessment of the patient which may include the  personal details such as name,age,occupation and  medical information like chief complaints,associated medical conditions ,contraindications for treatment, range of motion, muscle power and functional status etc.   It should definitely include the physiotherapy treatment chosen for that patient and a detailed description of the parameters of the treatment modality chosen such as the precise weight used for traction, the mode used in the interferential therapy, duration of the treatment,. positioning of the patietnt during treatment. apart from this the everyday progress of the functional status of the patient has to be recorded.
