Vitamin d deficiency with marked symptoms is an indication for injection form of vitamin d, If the symptoms are mild , vitamin d 60000 iu sachets are taken once a week for at least 15 weeks . Take calcium supplement as well daily,as it's needed for the absorption of vitamin d. Your vitamin B12 is low.Ideally with such low levels of vitamin B12 you need 1000-2000 microgram of cyanocobalamin daily
